Anyone who has ever needed a doctor knows how difficult the health care system can be to navigate, even on a small scale for a small ailment. When a person is elderly, chronically ill or suffers from another condition that makes caring for himself difficult, the system can present an overwhelming, unmanageable maze of options. This is where care management is crucial. Our care managers assume the responsibility of finding, arranging for and delivering the best combination of services for the person in need, so family, guardians, professional advisors, or others who are not experts in these matters, aren’t left with the enormous stress of managing these tasks on their own.

Most client relationships with Intervention Associates begin by speaking with one of them. We often receive calls from family members or guardians who have an urgent need regarding a loved one. Our care mangers work collaboratively, pooling their collective knowledge about the cost, quality and availability of services, to quickly solve the most difficult problems and deliver the best care possible.

What Our Care Management Services Include

Professional needs assessment and ongoing reports

In most cases, we begin with a needs assessment to determine and recommend the level and types of services required. This typically involves input from a loved one or guardian and a face-to-face evaluation of the individual's situation, environment and preferences. With ongoing care management, we also write monthly follow-up reports. Our care managers prepare all assessments and reports in a thorough narrative style, not in checklist format. Comprehensive care plan

Designed to help the individual maximize independence and quality of life, our care plan may include home health aides to assist with activities of daily living, transportation to appointments, light housekeeping and any number of other nonmedical services provided by our own trained employees. It may also include appropriate links to a variety of community and private resources—such as home meal deliveries and transportation services.

Recommendation of appropriate caregivers

The well-being of our clients depends on both their emotional and physical comfort. We strive to place home care staff that are most compatible with our clients and their loved ones. If for any reason a client prefers a different caregiver, their care manager arranges for an appropriate replacement.

Complete coordination and monitoring of in-home services

We identify, arrange for and schedule the multiple services provided for our clients; carefully oversee and monitor the quality of the services delivered; and assure our clients’ satisfaction. These activities are the care manager’s core responsibility.

Selection and monitoring of residential services

We assist in evaluating the appropriateness and quality of services within a nursing home, assisted living facility or other senior care residence, help families and clients select the right facility, and then monitor or supplement the care provided there.

Relocation assistance

We help move an individual to or from a retirement community, nursing home or other facility, as needed.

Patient advocacy and education

We follow clients from inpatient admission (to a hospital or nursing home) through discharge to help oversee their care. We also help educate clients on doctors’ orders and important details about their condition.

Care referrals

We can provide referrals to other care providers, such as doctors, adult day-care programs and outpatient rehab.

Financial, legal and insurance referrals

We assist in reviewing the diverse concerns of our clients and their families, and help connect them with available services: medical assistance and supplemental insurance like veterans and disability benefits, Medicare, prescription plans, etc.; qualified financial planners; elder care attorneys; and other professional advisors. We also regularly help clients decipher complicated medical plans and forms to help them maximize their benefits.

Professional crisis intervention and counseling

We are available to intervene in emergency situations where families (or others) are unable to cope with or cannot help someone who is confused, ill or suffering from addiction. We also provide individual and family counseling services.

Liaison

We regularly act as an intermediary to help communicate among clients, their family members (nearby or far away) and their health care providers.

Guardianship/power of attorney

We offer legal guardianship and durable power of attorney. Please ask us.
