Anyone who has ever needed health care knows how difficult the system can be to navigate, even on a small scale for a small ailment. Often, when a person is elderly, chronically ill or suffers from another condition that makes self-care challenging, his or her loved ones face an overwhelming maze of options and decisions. Care management can make all the difference.
What We Do at Intervention AssociatesOur care managers assume the responsibility of determining, arranging for and overseeing the best combination of services to help the person in need, so family, professional advisors or others who are not experts in these matters—and who are not trained or able to provide care themselves—aren't left with the enormous stress and uncertainly of handling these tasks on their own.

As a member of the National Association of Professional Geriatric Care Managers, Intervention Associates employs full-time, highly skilled and experienced care management staff. They work collaboratively, pooling their expertise about the cost, quality and availability of resources, to quickly solve the most difficult problems. Your care manager is your personal contact, working closely with you or your family members to deliver the best possible care for your loved one or client.
Professional needs assessment and ongoing reports
In most cases, we begin with a needs assessment to determine and recommend the level and types of services required. This typically involves input from a loved one or guardian and a face-to-face evaluation of the individual's situation, environment and preferences. With ongoing care management, we also write monthly follow-up reports. Our care managers prepare all assessments and reports in a thorough narrative style, not in checklist format. Comprehensive care plan
Designed to help the individual maximize independence and quality of life, our care plan may include home health aides to assist with activities of daily living, transportation to appointments, light housekeeping, meal preparation and any number of other nonmedical services. We deliver these services seamlessly, either through our sister company Caring Friends Home Care, or through another certified agency, upon request. An individual’s care plan may also include recommendations and links to government, community-based and private resources, as appropriate.
Complete coordination and monitoring of in-home services
We identify, arrange for and schedule the multiple services provided to our clients; carefully oversee and monitor the quality of the services delivered; and assure our clients’ satisfaction. These activities are the care manager’s core responsibility.
Liaison
We regularly act as an intermediary to help communicate among clients, their family members (nearby or far away) and their health care providers.
Health Care Navigation
Every day, in countless ways, we help clients and families overcome barriers to obtaining the best, most cost-effective health care and related services. Here are just a few of them:
Patient advocacy and education
We follow clients from inpatient admission to a hospital or nursing home through discharge to help oversee their care. We also help educate clients about their doctors’ orders and important details about their condition.
Health care and other professional referrals
We provide referrals to additional care providers, such as doctors, adult day-care programs and outpatient rehab. We can also make referrals to qualified financial planners, eldercare attorneys and other professional advisors.
Health insurance referrals and benefits
We assist our clients and their families in obtaining benefits they qualify for, such as medical assistance and supplemental insurance like veterans and disability benefits, Medicare, prescription plans, etc. We also regularly help clients understand their complicated medical plans and forms to help them maximize their existing benefits.
Residential services: selection, monitoring and relocation
We assist in evaluating the appropriateness and quality of services within a nursing home, assisted living facility or other senior care residence, help families and clients select the right facility, and then monitor or supplement the care provided there. We can also help move an individual to or from a retirement community, nursing home or other facility, as needed.
Professional crisis intervention and counseling
We are available to intervene in emergency situations where families or others are unable to cope with or cannot help someone who is confused, ill or suffering from addiction. We also provide individual and family counseling services.
Guardianship and other services
We offer legal guardianship and durable power of attorney as well as bill paying and related services. Please ask us.
