The care manager is a health and human services professional (typically with a degree in social work, nursing, psychology or related field) whose job is to carefully assess the needs of an individual; then plan, coordinate, deliver and monitor all care-related services. Locating, arranging for and overseeing the optimal combination of services to improve an individual's quality of life can be a daunting task—and a full-time job—for the loved ones of clients needing care. Our care managers are trained problem solvers with comprehensive knowledge of the quality and availability of services in the region we serve. Care management is not automatically included. There is an extra cost to have a care manager coordinate and supervise care. Anyone who has attempted to do it alone can attest to the value of leaning on a care manager and what that service means to the person in need, and to his or her family.
Along with the tasks described in the first question, home visits, assessments, travel time, report writing, recommendations and phone conferences are all included in the care manager's hourly rate.

We can help you find top-quality resources through this organization, through our care managers' personal knowledge and resources, and through other trusted professional networks.
Our care managers typically prepare a thorough needs assessment and make recommendations at the outset. For clients who choose ongoing care management, they also provide monthly follow-up reports (also known as case summaries). The client or family members identify the individual(s) who should receive this information. Every situation is different, but in all cases we adhere carefully to HIPPA regulations. Yes. We have more than 100 home care aides and all of them are our employees. Many have worked for Intervention Associates for 15 years or more.
Yes, everyone who enters your home meets our rigid quality credentialing standards.
A live-in caregiver remains in the home for at least 24 consecutive hours and requires eight hours of mostly uninterrupted sleep. An hourly employee must be awake and working during his or her entire schedule. If a client requires care or vigilance throughout the night, the live-in arrangement would not be an appropriate choice.
Intervention Associates provides services primarily in the five-county greater Philadelphia region: Montgomery, Philadelphia, Delaware, Bucks and Chester counties. We sometimes provide care management and concierge services out of state, but home health care services are limited to Pennsylvania. Our aides and social work assistants (concierges) may accompany clients on vacation and to family events.
Of course! Through our concierge services, a companion can transport and accompany you or a family member to medical appointments, social outings or even on vacation. Our social work assistants are not only available to help with everyday errands and engagements, but also provide a high level of intellectual, creative, and emotional support and stimulation to meet a wide variety of needs.
No, not necessarily. We often provide drivers and social companions for clients who need little other assistance. We provide bill-paying services though our accounting office at $45 per month for up to five checks and $3 for each additional check. These services can be contracted independently.
We invest a great deal of time, thought and collaborative effort into matching staff and clients. We understand that personal chemistry between caregiver and client/family is extremely important. Occasionally, for whatever reason, the fit may not feel quite right. If you or your loved one is not completely comfortable with the care manager or caregiver assigned, we will gladly make other arrangements.
We have comfortable, professional relationships with continuing care retirement communities, assisted living facilities, nursing homes and hospitals throughout the region. We frequently provide supplemental care/staffing for clients in these types of facilities.
We always prefer as much time as possible to choose the best fit for our clients; however, circumstances can often create an urgent need. We make every effort to provide services in whatever time frame we are given.
Legal guardianship is sometimes sought for an individual who has demonstrated full or partial inability to care for himself. Intervention Associates can assume this court-appointed role when no family or friends are available or appropriate to act as guardian. Durable power of attorney legally designates certain powers to others in the case of an incapacitating condition. It is drawn up with the help of an attorney and can be revoked at any time by the client. Intervention Associates functions in both medical and financial power-of-attorney roles. Our entire staff has a great deal of expertise. Several of our care managers specialize in special needs trusts and related issues, and have more than 20 years of experience in these areas. They are well known and respected by local attorneys and trust officers who work with special needs clients. Intervention Associates frequently assists clients in obtaining private insurance coverage and in navigating and securing medical assistance, including Medicaid waiver programs.
Collectively, our care managers have decades of experience with Medicare as well as Medicaid coverage and services. They help clients deal with these questions daily.
Often, clients who have served in the military are unfamiliar with the variety of benefits and services available to them through the United States Department of Veterans Affairs. New legislation and initiatives continually expand availability and access. Our care managers can help clients work through this complex system to obtain the benefits to which they are entitled.
