Our Services: An Introduction
Our services fall into three general categories: Care management, home care and concierge. In practice, the lines between these categories are not always so distinct. Most individuals and families have overlapping needs and our care managers deliver the services they require seamlessly.
Care Management
Care management encompasses all the activities required to properly plan for and deliver care to a person whose limitations prevent him from caring for himself. This function shifts the burden of determining what’s best for an individual—and the daunting task of finding and arranging for his care—away from family, friends and others, and places it with one of our highly skilled and credentialed professional care managers.
Among many other duties, the role of our care managers includes the following:
- Conducting a professional assessment (and regular reassessments) of the needs of the person requiring care
- Developing a detailed, individualized care plan
- Executing the care plan, by arranging, coordinating and overseeing all necessary services (usually multiple), including those provided by our own trained staff and by community-based providers
- Continuously monitoring all services provided
- Patient advocacy
Our care managers act as the “go-to” person and liaison for families. They provide medical referrals, help qualified clients obtain veterans assistance and disability benefits, connect clients and families with support groups, help decipher medical insurance, and much more.
Home Care
Home care refers to the kind of day-to-day assistance that medical insurance, including Medicare and Medicaid, either does not cover or only covers for a limited time after an acute ailment or hospital stay. People with Alzheimer’s disease, advanced chronic illness, physical and mental disabilities, and children with special needs often require help with activities of daily living (ADLs)—the basic everyday tasks that people normally accomplish to care for themselves. These include things like bathing, dressing, grooming and eating. This is where our home care services come in.
Unlike many other “home care” organizations, we employ our own carefully screened and trained certified nursing assistants and home health aides—more than 100 of them—supervised by registered nurses. Our home care staff is available for hourly, overnight and live-in assignments.
Concierge
People who need home care services frequently need help with related activities, such as grocery shopping, transportation, and someone to accompany them to doctor's visits. Our concierge services include these activities and just about anything else our clients need. Our skilled social work assistants can help with bill paying or shopping online. We even offer social companionship and drivers for activities such as attending movies and concerts or other outings.
